Referrals

We welcome referrals from professionals who work with young people aged 16 to 18 needing supported accommodation. Our referral process is clear, responsive, and aims to ensure the best match for each young person.

Eligibility Criteria:

​

• Young people aged 16 to 18

• Looked-after children or care leavers

• Medium to high support needs

• Transitioning from residential care or foster placements

• Willingness to engage in support, education or training, and life skills development
   

 

Referral Process:

​

• Initial inquiry via phone or email

• Submission of a referral form or placement request

• Review and assessment of the young person’s needs

• Site Visit by Young Person

• Offer of placement if suitable

• Planning meeting and agreed transition date
